Read moreFor any questions regarding our fees and charges, please feel free to contact our secretary or billing department during normal business hours. We participate with most major insurance companies and will submit all claims to yours. If we do not participate with your insurance company, payment will be requested at the time that services are rendered.
Co-pay is due at the time of service. Expect an additional $10 service charge for payments not rendered on time.
Please be prepared with a referral at the time of your visit or you may be asked to reschedule. If you have any special financial problems, we will make every effort to work out a mutually satisfactory payment plan.
