When you schedule your first appointment, our secretary will ask you to complete our standard questionnaire and deliver it to us by mail or email prior to your first visit. Please try to fill it out as completely as possible. These questions are imperative to the effective treatment of your condition. If any items are unclear, do not hesitate to contact our office for clarification. Upon arriving, give your name to the secretary and she will obtain any additional information we require. Please minimize the use of colognes and perfumes to respect our asthmatics. We also ask that patients do not eat or drink in the waiting room out of respect for patients with food allergies.
Cancellations and Late Arrivals
If you cannot keep your scheduled appointment, a two-day notice must be given to this office so that we may accommodate another patient or you will be subjected to a $25.00 fee.
Please try to make every attempt to arrive on time in respect to our other patients. If you are more than 15 minutes late, you may be asked to reschedule.
